Testimonials

Testimonial from Nancy Martens
"When I started having problems with my hand and arm, I spoke to Dr.Siddiqi about my concerns. I explained how my arm and hand felt like it was asleep most of the time. It caused me many sleepless nights with the tingling and numbness. I started to lose use of my hand. Dr.Siddiqi explained that it was carpal tunnel syndrome. He told me that carpal tunnel surgery was a simple surgery, and I agreed to have it done. I was so impressed, not only did my problems go away, but also, you can't even see a scar. Dr. Siddiqi did the surgery on my hand in a way that the incision was very small and the scar blended in with the lines on my hand. I have full use of my hand now and recovery was very short. I have seen and talked with others who have had this surgery elsewhere and they have ugly scars on their wrist. They were impressed when I showed them mine. I would do it again if I needed to".

Nancy Martens

Testimonial from Pat Hannon
Pat Hannon presented with bilateral hands numbness, pain, weakness which continued to worsen over time. Conservative care did not provide adequate relief and her symptoms were affecting her normal lifestyle. She decided to undergo carpal tunnel surgery which was performed with a mini-open, minimally invasive microsurgical technique. Pat Hannon has had considerable improvement of her symptoms following the surgery:

"Dr.Siddiqi operated on both of my hands with a good result. Thanks Dr.Siddiqi"
Carpal Tunnel FAQs

Q: I have a friend who had carpal tunnel surgery. He complains that his grip and ability to do things like unscrew a bolt to change the car oil is not as good as it was before. He also has an ugly scar. What alternatives do I have?

A: Some carpal tunnel syndrome (CTS) patients have complained after surgery about restrictions in their grip and pinch functions as well as tenderness in the palm, cosmetic problems and scar formation at the site of the incision. Fortunately, there is new surgical technique called “Mini-Open Carpal Tunnel Surgery.” The skin incision is only one centimeter long and is made in a very inconspicuous location. In clinical studies, this type of incision not only looked more aesthetic after healing, it also better preserved the grip and pinch function. It is also safer than endoscopic carpal tunnel surgery. The classic symptoms of CTS are night pain, numbness, tingling, pain in the fingers and hand and loss of strength. There may also be a sensation that the fingers are swollen or that there is poor circulation. If you suffer from any of these symptoms, you should seek treatment immediately as permanent nerve damage can result when nerve compression has occurred for prolonged periods of time.

Q: How can I find out if I have carpal tunnel syndrome?

A: Dr. Siddiqi will need to take a detailed history and examination. He then reviews this with nerve conduction studies & EMG’s of the hand to determine if you have carpal tunnel syndrome and how severe it is.

Q: If I have carpal tunnel, what treatment options are available for me?

A: Dr. Siddiqi will discuss what the best options are available for you. Initially, conservative care with splints and / or therapy may be all that you need.

Q: When is surgery considered for carpal tunnel syndrome?

A: If you are unable to tolerate the pain, numbness or your hands are getting weaker, then you may be a candidate for surgery. Dr. Siddiqi offers minimally invasive surgery options for carpal tunnel syndrome.

Q: What will happen if I do not have treatment for my carpal tunnel syndrome?

A: There is a risk that your hand weakness will continue to worsen and more permanent damage to the nerves and muscles of the hand can occur if carpal tunnel is not treated.
Carpal Tunnel Syndrome

What is Carpal Tunnel Syndrome?

Carpal tunnel syndrome or CTS is a common problem that
affects the use of your hand. You may be experiencing symptoms if you: Sometimes feel numbness, weakness, a tingling
sensation or experience pain in your wrist and hand, particularly at night.

The carpal tunnel is formed by the bones, tendons and
ligaments surrounding the median nerve. It supplies sensation to the thumb,
index and middle finger, and provides motion to the muscles of the thumb and hand. Carpal tunnel syndrome most often occurs when
this nerve becomes inflamed after being aggravated by repetitive movements.

These movements could include typing on a computer keyboard,
playing the piano or participating in daytime activities requiring you to bend
your wrist . The condition may cause narrowing or compression of this median
nerve (think “pinched” nerve).

The majority of people with carpal tunnel syndrome recover
completely. Only about one percent develop permanent injury. Avoid re-injury by
altering the way you perform repetitive tasks, the frequency with which you
perform the movements, and how much rest you get between these periods of
movement.

Neurosurgeons like Dr. Siddiqi of Houston’s Carpal Tunnel Clinic are medical
specialists trained to help patients who suffer from carpal tunnel syndrome,
among other illnesses. Dr. Siddiqi provides prevention, diagnosis, evaluation,
treatment, critical care and rehabilitation (operative and non-operative care)
of neurological disorders.

Dr. Siddiqi’s primary role is diagnosis, interpretation of
test results, and, when necessary, surgery. He is certified by the American
Board of Neurological Surgery (ABNS), the certification body which is
considered the “gold standard” in the field.

Endoscopic Carpal Tunnel Procedure

Carpal tunnel surgery can be performed using a Mini-open technique or with Endoscopic technique.
The endoscopic carpal tunnel release (Two portal technique) involves making two small incisions in the hand and forearm and the surgeon uses a camera to assist in viewing the carpal tunnel and perform the surgery. The surgery is not performed under direct visualisation of the carpal tunnel and median nerve, resulting in an increased risk of injury to the median nerve and increased risk of incomplete release of the carpal tunnel.

The possibility of nerve injury is slightly higher with the endoscopic technique, and injury to the median nerve or vessels or in cases of incomplete carpal tunnel release would need the use of open carpal tunnel surgery to directly visualise and adequately treat the condition. The patient’s recovery and return to work with the endoscopic technique may be quicker in some cases.

However, Endoscopic carpal tunnel release, though a relatively easier procedure, leads to neurovascular injuries more frequently than open surgery; thus open surgery is generally considered to be safer. The mini-open technique aliows the advantages of a minimally invasive technique, with a single small incision, and the safety of the open approach.

Non-operative care

Carpal tunnel syndrome is usually initially treated by immobilizing the wrist in a splint to minimize or stop pressure on the nerves. If this doesn’t work, patients are sometimes prescribed anti-inflammatory medications or cortisone injections in the wrist to reduce swelling.

Other conservative or non-surgical treatment for CTS may include rest and physical therapy. Many patients show considerable improvement with conservative, non-surgical care. If symptoms persist or worsen and significantly affect the patient's lifestyle, then minimally invasive surgical approaches to carpal tunnel may be discussed with the patient.

Microscopic Procedure

Microsurgical carpal tunnel release uses the mini-open surgical approach with the use of microsurgical techniques. The microscope provides magnified visualisation and a neurosurgeon versed in microsurgical techniques for peripheral nerve surgery uses fine microsurgical instruments to perform the surgery in a very delicate and safe manner under direct visualisation of the carpal tunnel and median nerve. Microscopic mini-open carpal tunnel release can be particularly useful in severe and difficult cases of carpal tunnel syndrome. It is also beneficial in patients who continue to have carpal tunnel syndrome after initial carpal tunnel surgery (e,g, from incomplete carpal tunnel release after endoscopic surgery) and patients who have developed a recurrence of carpal tunnel syndrome.

Patients generally return to work early. Most patients show significant improvement in numbness, pain, weakness of the hand. Microsurgical carpal tunnel release is thought to be safer and usually more cost-effective than endoscopic carpal tunnel release.

Mini-open Procedure

Mini-open carpal tunnel release has been the focus of recent attention for surgical decompression of carpal tunnel syndrome. It has been widely used as a surgical alternative for patients who are resistant to the conservative treatments.

This minimally invasive surgical technique uses a single one cm skin incision to allow a modified open decompression of the carpal tunnel. It allows complete direct visualization of the transverse carpal ligament and has lowered the occurrence of scar formation and wound discomfort. It is also safer than the endoscopic approach since the median nerve and the carpal tunnel is directly visualised during the procedure.

After surgery, the skin surface loses its biomechanical movements and elasticity even in the normal wound healing process. The limited skin incision occupies only a small area on the surface of the hand, providing more movement capability, elasticity and better appearance to the skin surface.

This mini -open, single skin incision technique for median nerve release can be used in the surgical treatment of carpal tunnel syndrome to achieve a better appearance. Clinical data has shown that this technique provides excellent results with respect to grip and pinch function, and cosmetic results.

Patients operated on with this technique have an easier postoperative experience compared with the standard incision.

Diagnosis of Carpal Tunnel Syndrome

Don’t wait for the pain to become unbearable. Seek medical assistance when you first notice persistent symptoms. Dr. Siddiqi will then conduct a comprehensive evaluation of your condition before recommending a course of treatment. Included in this examination will be an assessment of sensation, strength and reflexes in your hand.

If physical therapy and/or medication doesn’t provide satisfactory relief, certain diagnostic testing may be needed to determine if surgery is a viable option. The tests may include X-ray and Electromyogram/Nerve Conduction Studies (EMG/NCS).

X-ray – Shows the wrist bones and determines if any abnormalities are present that would lead to carpal tunnel syndrome.

Electromyogram/Nerve Conduction Studies (EMG/NCS) – Show how the nerves and muscles are working together by measuring electrical impulses along nerve roots, peripheral nerves such as the median nerve, and muscle tissue.

Causes of Carpal Tunnel Syndrome

Any repetitive and forceful grasping with the hands, bending of the wrist, and arthritis may cause carpal tunnel syndrome(CTS). CTS can also be caused by numerous conditions or diseases including pregnancy, diabetes, menopause, broken or dislocated bones in the wrist, and obesity.

Any repetitive motions that cause significant swelling or irritation of membranes around the tendons in the carpal tunnel can result in pressure and injury to the median nerve. This disrupts the flow of sensations from the hand up to the arm and to the central nervous system resulting in the sensation of numbness of the hands. The median nerve also supplies important muscles of the hand which may become weak in carpal tunnel syndrome.
